If you need to make a claim, please register your claim with us as quickly as possible, by calling us free on 0800 169 7733 between 8am – 7pm Monday to Friday. We will provide further advice over the phone and post out everything you’ll need to make your claim.
Need to complete a claim form?
If you have been advised by a Combined representative to complete a specific form in relation to your claim, you can download it rather than waiting for the post (speeding up your claim). Simply click on the form you need below and follow the instructions shown on the form:
Accident and/or Sickness New Claim Form
Complete the above form if you need to make a brand new accident or sickness claim.
Accident and/or Sickness Continuation Form
Complete the above form if you would like to continue an existing accident or sickness claim.
IMPORTANT: Once downloaded, the relevant form should be completed in full by you and your doctor and sent to us with your signature. On receipt of the completed form, we will review and assess your claim and policy coverage. We will then contact you confirming if/what benefits are payable. Please only use a continuation form if you need to continue an existing claim, otherwise use the new claim form indicated above.
